Job summary
Hampshire and Isle of Wight Healthcare NHS Foundation Trust have exciting opportunities that will enable you to gain more experience of supporting people to develop, retain or regain independent living skills that have been disrupted through the experience of living with mental illness.
Assertive Outreach Team to work collaboratively with Care Coordinators to co-produce good quality assessments and interventions to service users who are experiencing severe mental illness and find it difficult to engage with mainstream services. This is 22.5 hours a week.
Crisis resolution and home treatment team supports individuals who are experiencing acute mental health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible. Whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home. Service hours are Mon-Fri 9-5 or 2-10 with alternate weekends.
Main duties of the job

You'll work closely with patients, their families and carers, playing a vital part in ensuring everyone who uses our services has a positive and supportive experience. As part of a multi-disciplinary team, you'll work alongside nurses, activity coordinators, social workers and occupational therapists, offering you a unique chance to learn from different specialties every day.
