Job summary

Development Opportunity: Applications are welcomed from experienced nurses who demonstrate the potential to develop into a Band 7 Parkinson's Disease Nurse Specialist role through a structured development programme.

An exciting opportunity has arisen to join the Adult Ability Team supporting adults living with Parkinson's disease across East Staffordshire. Based in Burton upon Trent, the successful candidate will provide expert clinical leadership, specialist assessment, care planning and ongoing management for people with Parkinson's disease in the community, ensuring high-quality, person-centred care that enables individuals to live as independently and well as possible.

Working collaboratively with multidisciplinary colleagues across community, primary and voluntary sectors, you will act as a key worker, educator and advocate for patients, families and carers throughout their neurological journey.

This role offers the opportunity to influence service development, improve patient outcomes and provide specialist expertise within a well-established community multidisciplinary team

Main duties of the job

The Parkinson's Disease Nurse Specialist will provide expert assessment, care planning and ongoing management for people living with Parkinson's disease, supporting individuals and their carers throughout their condition. The role involves monitoring disease progression, managing symptoms, coordinating care across services, and delivering care mainly in peoples homes or care homes.

The post holder will contribute to service development and quality improvement, providing clinical leadership within the Adult Ability Team and ensuring care is evidence-based and aligned with best practice.

A key aspect of the role is educating and supporting patients, carers and healthcare professionals, whilst promoting understanding of Parkinson's disease and progressive neurological conditions across the local health community.

Working closely with neurologists, GPs, therapists, community and mental health teams, voluntary organisations and acute hospitals, the nurse will coordinate seamless care pathways and help people access the support and resources they need to maintain independence and quality of life.
