Job summary

An opportunity has arisen within the Rocket demand team to work within the COPD and Oxygen nurse specialist services.

Working across primary and secondary care the nurse specialist will act as the clinical expert and professional resource to the multi professional team, patients and carers within the specialist service.

The post holder will work alongside the rocket, oxygen and virtual ward teams to ensure the development and delivery of a high quality service for patients a requiring a respiratory nurse assessment. Providing education, assessment, monitoring and follow up for patients with COPD and those requiring home oxygen. This will include the management of a case load of patients and the provision of specialist advise within the clinical area. The role will include the provision of ward based assessments, advice and management of patients being discharged under the COPD/O2 team and home visits. To provide support and education for existing service users.

The post holder will act a clinical leader and role model for the ongoing development of clinical practice, research and standards of care. Effective liaison and good communication skills are essential to the post, a good knowledge of respiratory conditions and a degree module at diploma or degree level would be an advantage.

Having a valid manual driving licence is essential.

Main duties of the job

The main aspect of this role relates to providing an excellent level of care in the assessment and management of individuals referred to the COPD/Oxygen Nurse services.

Working autonomously in the community

To have knowledge of COPD and oxygen patients to be able to assist with virtual wards.

As part of the multidisciplinary team, the post-holder will be alert to national guidelines and clinical pathways for both new and chronic presentations of respiratory conditions

As such, the post holder will be reviewing patients referred to the Oxygen/COPD team within the acute setting. In addition, the post-holder will manage their own caseload of patients supported by a senior respiratory nurse or consultant.

The post holder will be confident in the management of individuals with respiratory conditions. This will include patient reviews in their own homes, delivering individualised care, facilitating lifestyle changes through education and support to patients and carers.

A Full driving licence is required for this post as community work is involved.
