Job summary
Stuart Crescent Medical Practice is seeking a compassionate, motivated and experienced Practice Nurse to join our team. This is an excellent opportunity to work in a supportive NHS GP practice committed to delivering high-quality, patient-centred care.
The successful candidate will provide evidence-based nursing care within the practice, supporting the delivery of essential and enhanced primary care services. The role includes chronic disease management, cervical screening, immunisations, wound care, health promotion and assisting the practice in achieving Quality and Outcomes Framework (QOF) targets and other contractual requirements.
You will work collaboratively with GPs, pharmacists, social prescribers and administrative colleagues to provide safe, effective and responsive care to our patient population.
Main duties of the job
The Practice Nurse will provide high-quality, patient-centred nursing care within a busy NHS GP practice. The role includes managing long-term conditions, undertaking cervical screening, administering childhood and adult immunisations and vaccinations, performing wound care, venepuncture, NHS Health Checks and health promotion activities. The post holder will work collaboratively with GPs and the wider multidisciplinary team to deliver safe, effective care, maintain accurate clinical records using EMIS Web, support Quality and Outcomes Framework (QOF) achievement, participate in clinical audits and quality improvement, and contribute to the delivery of high-quality primary care services in line with NHS and CQC standards.

We are a very friendly, family orientated and innovative practice with about 3300 patients which continues to grow. Our clinical team includes GPs, Practice Nurse, Clinical Pharmacists, Mental Health Practitioner, Social Link Prescriber, Health and Well-Being Coach and Care Coordinator all working together to provide the high levels of clinical excellence and support to our patients. We also thrive in staff training and progression.
We use EMIS Web clinical system with Docman 10 and have a great Administrative support Team with a triage system to manage workload.

Job description
Job responsibilities
- Deliver chronic disease management clinics (e.g. diabetes, asthma, COPD and hypertension).
- Undertake cervical screening in line with the NHS Cervical Screening Programme.
- Administer childhood and adult immunisations and vaccinations under PGDs/PSDs.
- Provide wound care, dressings, venepuncture, NHS Health Checks, and other agreed clinical procedures.
- Promote health and wellbeing through patient education and preventative care.
- Maintain accurate patient records using EMIS Web and ensure high standards of clinical documentation.
- Support the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.
- Participate in clinical audit, quality improvement and infection prevention and control activities.
- Work collaboratively with GPs and the wider multidisciplinary team, referring patients appropriately and contributing to the delivery of high-quality patient care.
- Maintain professional competence through continuing professional development and mandatory training, practising in accordance with the NMC Code, NICE guidance and CQC standards.

- - Experience undertaking clinical assessments and recognising when escalation or referral is required.
- - Experience maintaining accurate clinical records and using electronic clinical systems (e.g. EMIS Web).
- - Experience working collaboratively within a multidisciplinary healthcare team.
- - Experience supporting quality improvement, clinical audits and meeting NHS contractual requirements.
