Job summary

An exciting opportunity has arisen to join our Community Stroke and Neurological Rehabilitation Team as either a Band 3 Therapy Support Worker or Band 4 Technician, depending on skills, experience and competencies. A structured development pathway is available for Band 3 appointees to support progression to Band 4.

Our specialist service provides rehabilitation for adults living with stroke and a wide range of neurological conditions across community settings, including patients' homes, community hospitals and outpatient clinics. You will join a friendly, supportive multidisciplinary team committed to maximising independence and quality of life.

The role involves delivering rehabilitation programmes delegated by Occupational Therapists and Physiotherapists, supporting mobility, transfers, daily living activities, exercise programmes and self-management. You will monitor progress, provide feedback to therapists, support carers and work with health, social care and voluntary sector partners to achieve the best outcomes for patients.

We are seeking an enthusiastic individual with experience in a health, social care or rehabilitation setting, excellent communication skills, a flexible approach and a commitment to person-centred care. We offer regular supervision, appraisal, training, leadership development opportunities and support to develop specialist skills in stroke and neurological rehabilitation.

Main duties of the job

The purpose of the post is:

To carry out skilled interventions as delegated by senior Physiotherapists and Occupational Therapists

To assist patients to develop skills relating to mobility, transfers, activities of daily living, upper limb function, cognition, communication and community participation, within agreed competencies.

To monitor the patient's response to therapy intervention and report back on the outcome.

To encourage patients and carers in an active approach to regaining and maintaining personal independence.

This may be in clinics within community hospitals, in the community, or the patient's own home. This is may not be directly supervised by the therapist (e.g. practising mobility, exercises or activities of daily living).

To support safe ongoing rehabilitation in patients' homes and community settings.

About us

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.

Over 5,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

We work in people's homes, in community clinics and bases. Some staff work from one of our 10 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
