Job summary

A rare and exciting opportunity has arisen to join the HIV Community Clinical Nurse Specialist team, within the internationally recognised Chelsea and Westminster Hospital NHS Foundation Trust HIV directorate.

Acting as a link between Chelsea and Westminster Hospital NHS Foundation Trust HIV services and the community setting, you will holistically manage a caseload of people living with HIV, who may have complex physical and psychosocial needs. You will work within a small and dynamic team based from Chelsea and Westminster Hospital, regularly collaborating with other professionals, utilising telemedicine where appropriate and hot-desking from various sites. You will be responsible for visiting patients in their homes, or in other community-based settings, such as, GP surgeries, and drug and alcohol treatment centres.

The team aims to improve access to, and engagement with HIV treatment and care; to reduce unnecessary attendance and admission to healthcare services; and to support discharge planning where appropriate. They also seek to improve overall health, wellbeing and quality of life by empowering patients to take a fuller role in their healthcare and in addressing the wider determinants of their health. Apply today if you're eager for a new challenge.
