You'll deliver and coordinate specialist palliative and end of life care to people in their own homes across the 7am to 9.30pm service, including responding to SOS calls.

You'll assess, plan and evaluate care autonomously within agreed plans and your own competence, working alone with the CNS and the coordination centre behind you, and you'll run a safe, well-organised shift.

This is an enhanced practice role and the place where RGP grows its future specialists.

Main duties of the job

  • Carry out holistic assessment of patients and those close to them, and develop, deliver and evaluate individualised care plans.
  • Deliver fundamental and complex specialist interventions in the home, including symptom management within the RGP medicines framework, syringe driver care and, once competent, verification of expected death.
  • Respond to SOS and unplanned calls: assess, act within your competence, and escalate to the CNS, out of hrs advice line, or 111 when the situation needs decisions beyond your scope.
  • Deliver hands-on care when it is right for the patient, including on crisis response and unplanned care visits.
  • Communicate sensitive information with skill and care, supporting families before and after bereavement.
  • Keep accurate, contemporaneous records and hand over clearly across shifts and services.
  • Work autonomously within your competence and agreed plans, recognising the limits of your own competence and escalating appropriately.
