Job summary

We have an exciting opportunity for a Rehabilitation Assistant to join our multidisciplinary therapy team, delivering community-based rehabilitation to people with a range of conditions under the supervision of qualified Physiotherapists and Occupational Therapists.

This is a permanent, full-time role working Monday to Friday, 08:30-16:30. You will assess and manage an allocated caseload of clients with specific, predictable presentations, while also providing non-clinical support to qualified therapists.

You will work closely with colleagues across community and acute teams, GPs, social services and voluntary organisations to support patients' care, wellbeing and outcomes. You will also work towards and maintain the Intermediate Care competencies required for the role, contribute to team objectives and follow Trust and local lone-working procedures.

Please note: we are unable to offer sponsorship for this Band 4 role.

Main duties of the job

You will deliver planned rehabilitation interventions to clients in their own homes, day centres, the local environment and outdoors, under the supervision of a qualified therapist.

You will undertake holistic assessments within your level of competence, agree goal-focused treatment plans with clients and carers, monitor progress and escalate concerns or significant changes to the relevant therapist. You will manage an allocated clinical caseload, attend weekly multidisciplinary team meetings and contribute to safe, coordinated and effective care.

You will keep timely and accurate clinical records, report treatment outcomes through structured supervision, and use outcome measures to support progression of rehabilitation programmes. You will also assemble and adjust therapeutic equipment and demonstrate its safe use to clients and carers.

Effective communication is essential. You will work closely with patients, carers, therapy colleagues, health and social care professionals and voluntary organisations, adapting your communication to individual needs. You will attend training, complete relevant competencies and take responsibility for your ongoing development.
