Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Physiotherapist to join the newly established Burren Intermediate Care Beds Service.

The Burren is a new intermediate care service providing a dedicated step-down pathway for patients who are medically optimised for discharge from acute hospitals but require continued rehabilitation, recovery support, assessment and discharge planning before returning home. The service currently comprises 32 intermediate care beds alongside an additional bariatric pathway and plays a vital role in supporting patient flow across the BNSSG system.

This is a unique opportunity to be involved in the development and embedding of a brand-new service. As part of the founding therapy team, you will help shape pathways, establish ways of working and contribute to creating a high-quality multidisciplinary rehabilitation service from the outset.

The service has secured funding for its first year of operation and is being closely supported by system partners due to its important role in improving patient flow, rehabilitation outcomes and timely discharge from acute care settings.

Main duties of the job

As a Band 6 Physiotherapist, you will:

  • Provide specialist physiotherapy assessment, treatment and rehabilitation for patients with complex physical, medical and frailty-related conditions.
  • Work as an autonomous practitioner within a supportive multidisciplinary team.
  • Develop person-centred rehabilitation goals that maximise independence and recovery.
  • Facilitate timely discharge planning and safe transitions back into the community.
  • Support patients with a wide range of conditions including frailty, long-term conditions, neurological, medical and surgical presentations.
  • Work collaboratively with nursing, occupational therapy, medical, social care and community colleagues.
  • Contribute to service development, quality improvement initiatives and audit.
  • Provide supervision and support to junior staff, support workers and students.
  • Help establish and embed new clinical pathways and rehabilitation practices within the service.
