Are you a confident, motivated Adult Social Worker who thrives in a fast-paced multidisciplinary environment?
Manchester City Council is recruiting an experienced HDTA Social Worker to join their focused Hospital Discharge to Assess service for Central Manchester. Offering £30.00 per hour, this role involves working in close partnership with health colleagues to deliver strengths-based assessments, manage risk, and support citizens discharging from hospital into the community or bedded care placements.
About the Role & Service Area:You will be part of a busy MDT working at pace across hospital wards and bedded care settings for Central Manchester residents. Driven by the Our Manchester values-listening, trusting, and working together-you will deliver person-centred outcomes and manage statutory Adult Social Care responsibilities.
Key Responsibilities:-
HDTA & Strengths-Based Assessments: Complete comprehensive, strengths-based assessments and support plans for citizens requiring short-term reablement or long-term care following hospital discharge.

Mental Capacity & Best Interests: Conduct statutory Mental Capacity Act (MCA) assessments and coordinate multi-agency Best Interest decisions.

Adult Safeguarding: Lead on complex adult safeguarding enquiries and manage risk within a health and social care environment.

Duty Rota & MDT Collaboration: Participate in the team duty rota, work collaboratively alongside health partners, and advocate in the best interests of citizens.

Qualifications & SWE: Recognised Social Work qualification and active registration with Social Work England (SWE).

Hospital / HDTA Experience: Proven experience in statutory Adult Social Care (Hospital Discharge, HDTA, Reablement, or Locality teams).

Key Skills: High-level risk management, strong communication skills, confidence to challenge constructively in the best interest of citizens, and the ability to work at pace.

Competitive Rate:£30.00 per hour (35 hrs/week).
