Job summary

We are looking for an enthusiastic Practice Nurse, on either a full-time or part-time basis, to join our established nursing team. The successful candidate will have experience in chronic disease management and be confident in carrying out chronic disease reviews.

This is a varied and rewarding role within a friendly, hardworking and supportive team. We pride ourselves on providing a high standard of care and excellent service to our patients.

We are looking for someone who is caring, approachable and great with people, with excellent communication and interpersonal skills. A positive and friendly manner is important, together with good IT skills and the ability to work effectively as part of a busy practice team.

Main duties of the job

The successful candidate will provide high-quality nursing care to our patients, including the management and review of long-term conditions such as asthma, COPD, diabetes and hypertension. It is expected that the successful applicant will take responsibility for at least one area of long-term condition management, according to their skills and experience.

The role will also include a range of general Practice Nurse duties, including wound care and management, cervical screening, ECGs, Doppler assessments, childhood immunisations and travel vaccinations.

The Practice Nurse will also support the delivery of the practices seasonal and routine vaccination programmes, including influenza, pneumococcal, shingles and COVID-19 vaccinations.

ACCOUNTABLE TO: Nursing Lead GP Partner

DUTIES AND RESPONSIBILITIES

The Practice Nurse will work as part of the multidisciplinary team to provide high-quality, patient-centred nursing care.

A key part of the role will be the management and review of patients with long-term conditions, such as asthma, COPD, diabetes and hypertension. The postholder will take responsibility for at least one area of chronic disease management, according to their skills and experience, and contribute to the achievement of relevant QOF requirements.

CLINICAL DUTIES

The role will include a range of general Practice Nurse duties, including:

  • Chronic disease reviews and management.
  • Wound care, dressings and leg ulcer management.
  • Cervical screening.
  • Venepuncture and specimen collection.
  • ECGs and Doppler assessments.
  • Blood pressure and other routine clinical observations.
  • Suture and clip removal.
  • Ear irrigation, where appropriately trained.
  • Supporting minor surgery and other clinical procedures.
  • Responding appropriately to medical emergencies.

VACCINATIONS AND IMMUNISATIONS

The postholder will support the delivery of the practices vaccination programmes, which may include:

  • Childhood immunisations.
  • Travel health advice and vaccinations.
  • Seasonal and routine vaccinations, including influenza, COVID-19, pneumococcal and shingles.
  • Maintaining appropriate vaccine records and cold-chain requirements.

PROFESSIONAL RESPONSIBILITIES

The postholder will:

  • Maintain current NMC registration and work within their professional scope of practice.
  • Maintain appropriate clinical competencies, mandatory training and continuing professional development.
  • Maintain accurate clinical records and patient confidentiality.
  • Follow safeguarding, infection control, health and safety and other practice policies.
  • Work collaboratively with the wider practice team and other healthcare professionals.
  • Participate in audit, quality improvement and practice development where appropriate.
  • Help maintain clinical equipment and appropriate Treatment Room stock levels.

GENERAL
