Job summary
We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our Team for 33 hours a week. This role is pivotal in providing high-quality, patient-centred care in a fast-paced, supportive general practice environment.
As an ANP, you will lead on delivering undifferentiated primary care to a varied patient population, working independently but with access to clinical support from the practice team. The successful candidate will demonstrate excellence in clinical assessment, decision-making, and prescribing, with a commitment to upholding the standards of care and safety expected by Mayflower Medical Group.
Main duties of the job
Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, and advise patients face to face, ensuring the use of evidence-based practice. The role encompasses the following areas:
Undifferentiated Patient Care within an AM and PM session
Patient follow up - respond to previous case management
Autonomous Practice - as per NICE guidelines
Case Management - Ensuring pathways meet the needs of our service
Prescribing - Independant prescribing

Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.
Case Management
Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.
Prescribing
Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.
Home visiting
undertake home visiting as and when required, including triage of home visits.
Leadership and Guidance
Mentorship
Provide mentorship and supervision to nursing staff, trainee advanced practitioners, and other healthcare professionals to enhance team competencies.
Professional Development
Engage in continuous professional development to keep abreast of medical advancements and to further develop clinical skills.
Research and Quality Improvement
Research Participation
Participate in clinical audits and research activities to contribute to the evidence base for practice.
Quality Assurance
Implement quality improvement initiatives and ensure compliance with health care standards and best practices in patient care.
Educational Activities
CPD Activities Facilitate and participate in Continuous Professional Development (CPD) activities within the practice.
