Job summary
An exciting opportunity has arisen for the recruitment of a Community Staff Nurse based at South Shore Nursing Team. The advertised role is a Temporary fixed term position (30.00 hours per week).
It is essential that you have a full UK driving licence and access to a car for this Community Nursing role.

NMC Level 1 Registration (Adult Branch) & Enhanced DBS clearance required.
Car driver essential.
Main duties of the job
The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients' homes or clinic settings.
To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.
The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.
To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.
