Job summary
We are looking for enthusiastic and passionate ACPs with varying experience to join our GP Federation Channel Health Alliance Bank Care Service.
Would you like to work in Urgent Treatment Centres, Primary Care Enhanced Access Services allowing patients increased access to same care for minor illness who value your skills and knowledge and where you have time to provide high quality care.

Main duties of the job
Role Requirements - Working independently you will provide a high quality of service to patients and their families, ensuring their care and treatment needs are met.
You will need to have ACP experience of working with adults with minor illness, long terms conditions and complex health needs. You will be exceptionally organised with a high-level of attention to detail. You will naturally possess excellent inter-personal skills, and an ability to consult and positively engage with key stakeholders across the organisation.
Job Description
You will work as a Bank ACP and ensure the safe and effective delivery of a high standard of individualised care across flexible sessions Monday to Saturday, and where required in collaboration with the Primary care Multidisciplinary Team.
Part of the EA role will require some Home Visiting Consultations.
You will work within a holistic model to ensure care and treatment needs are met
