Job summary

Are you a caring, dynamic, and motivated person interested in working to support patients in their own home? We are looking for a Rehabilitation Assistant to join the Intermediate Care team, which is part of Responsive Services team in First Community Health and Care.

The Rehabilitation Assistants work collaboratively with the multidisciplinary team including Physiotherapists, Occupational Therapists, Nurses, other Rehabilitation Assistants and Administrative Staff. The Intermediate Care team provides short term care packages and rehabilitation to patients who have recently been discharged from hospital or those at risk of being admitted to hospital. The post of Rehabilitation Assistant involves providing care visits and rehabilitation in the patients own home under the direction or our nurses and therapists.

Due to nature of the role it is essential that applicants are able to drive and have access to a car suitable for business use. The use of public transport to complete this role is not possible due to the area that we cover.

All staff undertake an induction programme. At First Community Health and training and development opportunities are supported through an annual appraisal process and in-house training.

The service operates from 08:00-20:00 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis.

  • Experience of working autonomously and as part of a team
  • Must be able to manage time effectively
  • A willingness to undertake competencies to enhance skills in the role.
  • You must be a car driver and have access to a car
