Job summary

It is an exciting time to join the Haemoglobinopathy team at Sandwell and West Birmingham NHS Trust as we look forward to a new working model of integrating community and hospital Sickle Cell and Thalassaemia Service.

The team are looking for a friendly and dynamic nurse to join the team in a liaison nurse role, an integral part in facilitating the transition between paediatric services, but increasingly bridging the gap between hospital and home too.

We are looking for someone with experience of caring for people with haemoglobinopathy conditions or awareness of the challenges they face. Coordinating and participating in regional MDTs, teaching and raising awareness throughout hospital and community.

This role involves visiting patients in their own homes to deliver a holistic approach to care. As it requires working within the community, a full UK driving licence and access to a personal vehicle are essential.

Supporting the Lead nurse with service development.
