Job summary

Somerset NHS Foundation Trust is seeking a Consultant Obstetrician and Gynaecologist with a special interest in fertility to join our team, based at Yeovil District Hospital.

This is a substantive 10 PA post combining a general Obstetrics and Gynaecology role at Yeovil with a cross-county fertility remit. The successful candidate will provide fertility clinics across both Yeovil District Hospital and Musgrove Park Hospital, working closely with our Fertility Nurse Specialist and endometriosis team to support a joined-up fertility service across Somerset.

The role will include assessment and management of subfertility, fertility ultrasound and follicular tracking, ovulation induction and monitoring, appropriate investigation of tubal and uterine factors, and counselling and onward referral for assisted conception where required. The post holder will also contribute to the development, governance and quality improvement of fertility services across the county.

Alongside this specialist work, you will contribute to general obstetric and gynaecological care at Yeovil, including outpatient, theatre and emergency activity, and participate in the consultant 1:8 on-call rota.

Main duties of the job

*Provide high quality obstetric and gynaecological care across inpatient, outpatient and emergency settings within Yeovil Hospital and Musgrove Park Hospital.

*Participate in the 1:8 Obstetric & Gynaecology on-call rota, including twice daily consultant led ward rounds on Labour Ward at Yeovil Hospital.

*Lead and support general gynaecology clinics, antenatal clinics, day surgery and elective caesarean lists.

*Contribute to the departmental teaching, supervision and appraisal of resident doctors and other members of the multidisciplinary team.

*Engage actively in clinical governance, audit, quality improvement and service development.

*Collaborate with multidisciplinary teams across Yeovil Hospital and Musgrove Park Hospital to support safe, effective and patient-centred care.

*Work with service managers and clinical colleagues to manage their caseload flexibly and ensure that clinical, national and local priorities including referral to treatment waiting time standards are achieved.

Job description

Job responsibilities

*Provide Consultant-led fertility clinics across both Yeovil Hospital and Musgrove Park Hospital as part of a cross county fertility service.

*Assess, investigate and manage patients presenting with subfertility, including both female and male factor infertility and formulate appropriate individual management plans.

*Undertake and interpret gynaecological ultrasound relevant to fertility care, including transvaginal ultrasound and follicular assessment / tracking.

*Arrange and interpret appropriate investigations for tubal and uterine factors, including HSG and HyCoSy / saline infusion sonohysterography (SIS) in accordance with local pathways and individual competence.

*Assess and manage ovulatory dysfunction, including polycystic ovary syndrome and provide appropriate ovulation induction and monitoring.

*Counsel patients regarding fertility treatment options, including the benefits, risks and limitations of assisted reproductive techniques and arrange appropriate onward referral to tertiary fertility / assisted conception services where required.

*Recognise, assess and manage complications associated with fertility treatment including Ovarian Hyperstimulation Syndrome (OHSS).

*Support appropriate fertility preservation pathways and onwards referral for patients undergoing medical or surgical treatment that may affect fertility.

*Provide clinical leadership and oversight for fertility care and pathways across Somerset. Working collaboratively with colleagues across both acute sites.

*Work closely with the Fertility Nurse Specialist based at Musgrove Park Hospital, providing clinical supervision, support and professional leadership as appropriate.

*Work closely with the Endometriosis Leads and wider multidisciplinary team to support the assessment and management of patients with endometriosis associated sub-fertility, contributing to the fertility element of the Trust's accredited endometriosis service.

*Work collaboratively with the relevant specialities and external providers, including tertiary fertility services to provide coordinated patient care.

*Contribute to the development of governance, audit and quality improvement of fertility services across Somerset.

  • *Relevant CCT or equivalent (or within 6 months of interview).
  • *Completed advanced training in the management of subfertility / reproductive medicine (ATSM / SITM or equivalent).

Desirable

  • *An appropriate higher degree of qualification (MD, PHD or equivalent).
  • *MSC or equivalent higher qualification.
  • *Educational supervision qualification.

Experience

  • *Depth and breadth of clinical experience in the assessment and management of subfertility.
  • *Experience and management of both female and male factors infertility.
