Responsibilities include leading the care planning process, transitioning residents smoothly, monitoring resident well-being, maintaining care standards, and practicing safe work systems. This role aligns with our values of caring, support, honesty, respect, and accountability. If you meet these standards and possess the necessary qualifications, particularly an NVQ 3 in care, with two years experience in a senior position and effective communication skills, we invite you to join us.

Other responsibilitieswillinclude:
- Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
- Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
- Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
