South and West Herts Older People & Sensory Services Team are looking for an agency Social Worker

Are you an experienced and dedicated Social Worker looking for a rewarding role within Adult Care Services?

Hertfordshire County Council is seeking a skilled professional to join the Older People & Sensory Services Team.

This is a fantastic opportunity to make a positive impact on the lives of older adults and individuals with sensory impairments, helping them maintain their independence, safety, and well-being.

Key Responsibilities:

Manage a varied caseload involving older people and individuals with sensory needs.
Conduct comprehensive Care Act assessments, support planning, and mental capacity assessments. Complete robust safeguarding adult investigations and implement risk-management strategies.
Work collaboratively with multi-disciplinary teams, health colleagues, and voluntary agencies. Deliver person-centered interventions that promote independence and choice.

Requirements
Social Work England registration.
Recognised Social Work qualification (Degree, DipSW, CQSW, etc.).
Significant post-qualifying experience working within adult social care, preferably with older people or physical/sensory disabilities.
Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.
Excellent communication, assessment, and report-writing skills.
Full UK driving license and access to a vehicle for community visits.
