Job summary

We are looking for an enthusiastic and motivated Band 4 Assistant Practitioner to join the Burren Intermediate Care Beds Service on a 12-month fixed-term basis. This is an exciting opportunity to be part of a developing service and contribute to shaping and delivering high-quality rehabilitation and recovery support.

The service provides rehabilitation, recovery and discharge planning support for adults who no longer require acute hospital care but need further assessment, rehabilitation, reablement and multidisciplinary support before returning home or moving to a more appropriate care setting.

Working as part of an integrated multidisciplinary team, you will deliver delegated rehabilitation and therapy interventions, supporting individuals to achieve their goals, maximise independence and progress towards a safe and timely discharge. The role supports the BNSSG Discharge to Assess pathway and Home First approach, with a strong focus on person-centred rehabilitation and recovery.

Main duties of the job

As an Assistant Practitioner, you will:

  • Deliver delegated rehabilitation, therapy and care interventions within agreed competencies and scope of practice.
  • Undertake delegated assessments and treatment activities under the direction of registered practitioners.
  • Support rehabilitation programmes focused on mobility, transfers, functional activities, exercise and independence with daily activities.
  • Monitor patient progress and escalate changes in condition, concerns or risks appropriately.
  • Support individuals, carers and families to understand rehabilitation goals and treatment plans.
  • Contribute to rehabilitation, recovery and discharge planning as part of the multidisciplinary team.
  • Work collaboratively with Physiotherapists, Occupational Therapists, Speech and Language Therapists, Nurses and other health and care professionals.
  • Maintain accurate patient records and work in accordance with relevant clinical governance and organisational requirements.
  • Participate in supervision, training, competency development and service improvement activities.
