This to work within a Learning Disability Team
Role Overview:
Cambridgeshire County Council is seeking an experienced Social Worker to join the Huntingdon Learning Disability Partnership Team. The team provides specialist support to adults with learning disabilities, working collaboratively with individuals, carers, families, health professionals and partner agencies to promote independence, wellbeing and positive outcomes.
Key Responsibilities:
Complete strengths-based assessments under the Care Act 2014. Develop, implement and review person-centred care and support plans. Undertake safeguarding enquiries and risk assessments. Complete Mental Capacity Assessments and Best Interest decisions. Support adults with learning disabilities and complex needs. Manage complex caseloads and coordinate multi-agency interventions. Prepare reports for safeguarding processes, panels and legal proceedings. Complete Community Deprivation of Liberty applications to the Court of Protection. Work closely with Community Learning Disability Nurses, Psychologists, Occupational Therapists and other professionals within the Partnership Team. Complete carers' assessments and provide support to carers.
