Job summary

Are you a registered nurse looking for a new challenge in a community setting?

An opportunity has arisen for a dynamic Health Visitor to join the Oxford City South locality on a part time basis. The post is to work in the integrated 0-19 Childrens and Young People Public Health Service here in the Iffley/ Cowley/ Leys area. The new service brings together Health Visiting, School Health Nursing, and the Family Nurse Partnership, delivering the Healthy Child Programme (HCP) to provide an innovative and high-quality service that helps children, young people and families to thrive.

Health visitors, community public health nurses (previously known as community staff nurses) and community public health associates (previously known as community nursery nurses) will now be working with children up to 8 years of age where there are known concerns or vulnerabilities and will provide health related episodes of care to support with areas such as behaviour, diet, and sleep. There has also been an introduction of a new four-year health and development review to support school transition and address any school readiness concerns.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Main duties of the job

• Leading the delivery of the Healthy Child Programme completing the 5 mandated contact with the addition of the new 4 year health and development review to support school readiness and transition.
• Responsible for a skill mix team delivering public health service initiatives to a defined population.
• Deliver services to children and families within homes, and community settings. Clinical activities will include assessing, planning, implementing and evaluating family focused care and delegating to CPHA and CPHN when appropriate.
• Develop multi agency relationships and work closely with other agencies including the voluntary sector.
• Establish and maintain partnerships with families' and other services and agencies in multi-agency and integrated working processes, identifying the health needs in a defined locality and targeting resources to address these.
• Assess, plan, implement and evaluate individual/family focused/group programmes of care, working in partnership with other agencies, including early years' provision.
