Job summary
You'll support people receiving rehabilitation care, helping them maintain their wellbeing, independence and quality of life. Working as part of a caring and supportive team, you'll help deliver person-centred care that focuses on what matters most to each individual. Your contribution will make a real difference to patients' experiences by providing practical support, encouragement and compassionate care every day.
Main duties of the job
What you'll do:
- Support patients with care activities in line with their individual care plans
- Carry out agreed clinical tasks, such as observations, blood pressure monitoring and specimen collection following training and competency assessment
- Communicate effectively with patients, carers and healthcare professionals
- Record patient information accurately on electronic systems and support basic administrative tasks
- Help maintain a safe, organised environment, including preparing equipment and managing stock levels
- A Level 2 qualification in Adult Health and Social Care, or equivalent experience
- Experience supporting people in a healthcare, homecare or care home setting
- Strong communication and interpersonal skills, with a caring and respectful approach
- Ability to work independently and as part of a team
- Willingness to complete the Care Certificate during induction if not already achieved
