Main duties of the job
- Deliver diabetes reviews and ongoing management.
- Provide patient education and lifestyle advice.
- Monitor and optimise diabetes treatment in line with national guidelines.
- Undertake chronic disease management clinics.
- Administer vaccines and immunisations.
- Perform cervical screening, wound care and health promotion activities as appropriate.
- Maintain accurate clinical records and contribute to quality improvement initiatives.

The successful candidates will play a key role in delivering high-quality nursing care with a particular focus on diabetes management. There are two positions available: 1x 30 hours per week and 1 x 20 hours per week.

- Registered Nurse with current NMC registration.
- Diploma in Diabetes Care.
- Experience working in primary care.
- Competent in managing patients with diabetes, including initiating
- and reviewing treatment plans.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary
- team.
Desirable
- Experience in managing other long term conditions (e.g. Asthma,
- COPD).
- Cervical cytology qualification.
- Immunisation and vaccination training.
- Travel health and wound care experience.
- Knowledge of SystmOne.
