Job summary

The main aim is to improve the outcomes for patients from their medicines and reduce the risk harm. Part of this will involve improving medicines safety and medicines management processes across the practice, such as contributing in actioning MHRA alerts and reconciling of medicines. You will carry out structured medication reviews to optimise medication for complex patients.

Main duties of the job

The following are the core responsibilities of the Prescribing Clinical Pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

a. To support the practice with all aspects of medicines optimisation

b. Patient facing or telephone medication reviews

c. Structure medication reviews with direct patient contract

d. Management of long-term conditions

e. Reviewing systems for safer prescribing

f. Contributing to repeat prescriptions authorisation and reauthorisation

g. Actioning acute prescription requests

h. Contributing to achievement of QOF and locally commissioned quality improvement schemes

i. Undertaking clinical audits

j. Implement prescribing changes

Collingham Church View Surgery and Thorner Surgery form a well-established, five-partner, semi-rural General Practice dedicated to delivering high-quality, patient-centred healthcare to the local community.

The practice serves approximately 10,000 patients across two recently renovated premises, both of which have been refurbished to a high standard and are equipped with modern facilities designed to support the delivery of contemporary primary care services. The surgeries provide a welcoming and accessible environment for patients, with consultation rooms, treatment areas, and clinical equipment that meet the evolving needs of modern General Practice.

Supported by a comprehensive Primary Health Care Team, the practice offers a wide range of medical services and works collaboratively with community healthcare professionals to ensure integrated and holistic patient care. The multidisciplinary team includes GPs, nurses, healthcare assistants, administrative staff, and allied healthcare professionals who work together to provide safe, effective, and responsive services.

Committed to innovation and efficiency, the practice is fully computerised and operates a paper-light system, utilising modern digital technologies to enhance clinical care, streamline administrative processes, and support effective communication between healthcare professionals and patients.
