The Role:As a full-time Band 7 therapist specialising in frailty, you will work with doctors, advanced practitioners, nurses, therapists and pharmacists to support frail patients both in the hospital and in the community. Promoting a home first approach to the service.

Main duties of the job
Exciting Opportunity! Join the Frailty Team as a Specialist Therapist
Are you passionate about community/hospital dedicated to the care of frail patients, promoting a home first approach. We're seeking a dynamic therapist at Rochdale Infirmary to support residents of Heywood, Middleton, and Rochdale (HMR).
You'll play a key role in delivering the frailty service, ensuring timely, appropriate care is provided in patients' own homes.
You'll work closely with patients and carers to manage health needs, promote self-care, and develop personalized plans. Collaborating with multidisciplinary teams, you'll help shape high-quality, patient-centered services aligned with trust goals.

Qualifications & Professional Registration
Essential
- Registration with the appropriate professional body ie NMC/HCPC
- Degree qualification or equivalent
- Recognised teaching/mentor qualification
Desirable
- Previous experience of team leadership
- APLS / ALS
- Level 7 clinical skills
- V300 (non medical prescribing) or willingness to undertake
Knowledge, Training & Experience
Essential
- Substantive post registration experience
- Extensive knowledge/experience in urgent and or community setting
- Knowledge and understanding of local health and social care pathways
- Understanding of long term conditions including frailty and end of life processes
- Evidence of planning and leading service development
Desirable
- Previous experience or involvement in the development of service policies and protocols
Skills & Abilities
Essential
- IT skills
- Promotes and support own and others CPD
- Ability to travel to different geographical locations across the trust footprint in line with service needs.

- Full UK driving license with willingness to use own car for work purposes
