Job summary
We are seeking an enthusiastic Advanced Care Practitioner to join our friendly, patient-centred and forward thinking team based in Louth.
The successful applicant will work autonomously to assess, diagnose and treat patients across all age groups, providing high-quality clinical care both within the practice and in patients homes where required.

Main duties of the job
- Provide Provide high-quality clinical assessment, diagnosis and treatment.
- Manage a caseload of patients independently.
- Conduct a mix of face-to-face and telephone consultations.
- Undertake home visits where clinically necessary.
- Work collaboratively within the multidisciplinary team.
- Maintain accurate clinical records using SystmOne (training available if required).
- Contribute to practice initiatives and continuous improvement.

- Ability to work autonomously and as part of a team.

- Primary care or acute care experience
- Independent Prescribing qualification (V300)
- Masters level Advanced Practice qualification (or working towards)
