This role will be employed through Oxleas on behalf of the Bexley system to support neighbourhood delivery. The role will lead on the implementation of the priorities as set out in the Bexley Wellbeing Partnership Neighbourhood Plan and support the development of the three Local Care Networks with their delivery plan - with a particular focus on improving outcomes and reducing health inequalities.
This role will be a key member of the Neighbourhood Transformation Team, which will support delivering transformational change for the Bexley Wellbeing Partnership - working with the three Local Care Networks and the four Primary Care Networks. This will ensure that the partnership better joins up health and care support, improve people's health and wellbeing and close the inequalities gap.
The post holder will play a key role in building collaborative relationships between Local Care and Primary Care Networks and Bexley Care and wider stakeholders within the health and care system and local communities.
Main duties of the job
- The post holder will be a part of the Neighbourhood Transformation Team, led by the Head of Neighbourhood System Transformation working the Local Care Networks and will contribute towards the effective delivery of the partnership's objectives and the Neighbourhood Plan.
- Enable the development of integrated approaches across health, care, and wider partners by facilitating and promoting innovation and transformation to improve outcomes with Local Care and Primary Care Networks.
- Working with key stakeholders, VCSE, Social Housing, Public Health, Clinical Leads (Clinical Directors), community and mental health providers.
- Provide the day-to-day project and programme management support to the LCN and associated programmes.
- Be the lead for the Health Inequalities working with Clinical Leads for Health Inequalities with Public Health in identifying and developing business cases to address reducing health inequalities.
- Build and strengthen working relationships with key stakeholders, spanning beyond the core partner, that make up the Bexley Wellbeing Partnership and into our local communities.
- Ensuring appropriate involvement and engagement with service users, local communities, carers and their families and frontline staff in the development and delivery of service redesign - working with partnership engagement team.
- Role modelling the collaborative leadership behaviours and the principles as set out in the Bexley Wellbeing Partnership Memorandum of Understanding.

Job description
Job responsibilities
The post holder will be a part of the Neighbourhood Transformation Team, led by the Head of Neighbourhood System Transformation working the Local Care Networks and will contribute towards the effective delivery of the partnerships objectives and the Neighbourhood Plan.
Enable the development of integrated approaches across health, care, and wider partners by facilitating and promoting innovation and transformation to improve outcomes with Local Care and Primary Care Networks.
Working with key stakeholders, VCSE, Social Housing, Public Health, Clinical Leads (Clinical Directors), community and mental health providers to identify and respond to the needs and priorities for populations through quantitative and qualitative intelligence.
Provide the day-to-day project and programme management support to the Local Care Network and associated programmes.
Work collaboratively with project and programme managers across partner organisations to encourage and develop the sharing of good practice and interdependencies between the Bexley Wellbeing Partnership workstreams which are identified and planned for.
Be one of the dedicated lead managers for delivery in the 3 Local Care Networks.
Lead on specific projects, commissioning and service development or transformation within the Local Care Network to support the delivery of key outcomes.
Be responsible for development and delivery of the Local Care Network Plans.
Be the lead for the Health Inequalities working with Clinical Leads for Health Inequalities with Public Health in identifying and developing business cases to address reducing health inequalities adopting co-production approaches with local communities.
Create a stronger emphasis on the golden thread within Bexley Wellbeing Partnership Neighbourhood Plan; including tackling inequalities and securing good outcomes, workforce, prevention, joined up care, working with the community, sharing resources, data driven and Local Care Network focussed.
Build and strengthen working relationships with key stakeholders, spanning beyond the core partner, that make up the Bexley Wellbeing Partnership and into our local communities.
Provide expertise and leadership in translating strategy and policy, good practice from across the UK and globally and lessons learned from work done to date to develop, transform and improve local services and support for residents in Bexley.
Ensuring appropriate involvement and engagement with service users, local communities, carers and their families and frontline staff in the development and delivery of service redesign working with partnership engagement team.
Role modelling the collaborative leadership behaviours and the principles as set out in the Bexley Wellbeing Partnership Memorandum of Understanding.

The postholder will be expected to foster strong working relationships with a wide range of internal and external stakeholders, cultivating a supportive and safe environment in which strengths can be shared, challenges made and the impact of decisions on the whole system reviewed and understood.
The post holder will be required to use highly developed communication skills to provide and receive highly complex, sensitive, and often contentious information and data to various stakeholder and internal audiences. This will include drafting and presenting briefing papers and presentations for both internal and external use.
Use well developed and adaptive communication and influencing skills to engage a wide range of stakeholders.
Provide regular written and verbal briefings and communication to various forums both within programme governance and within partner/stakeholder organisations. Including, contributing to the Bexley Wellbeing Partnership microsite, lunch and learn sessions and other communication outputs.
Develop and maintain mechanisms to facilitate the involvement of people with lived experience and their carers/families in the development and continuous improvement of services.
Maintaining good links with voluntary sector and other non-statutory partners (e.g., education, care home sector) as relevant to the scope of Bexley Wellbeing Partnership workstreams and projects.
Use experience-based co-production approaches to ensure the voice and experience of Bexley residents is at the heart of what we do.
Utilise negotiation skills to improve and develop the integrated approach and cross-boundary working for the benefit of the population of Bexley.
Provide expertise in the continued development of relationships across the system, acting as an ambassador for Bexley Wellbeing Partnership.
Financial and Physical Resources
The postholder will have responsibilities for ensuring that resources are deployed effectively to deliver the programme.
The postholder will have specific responsibilities for overseeing the Health Inequalities Fund allocations to Local Care Networks.

This role is responsible for supporting rigorous and robust transformation and improvement programmes; strategy, project and programme management including planning, reporting, managing risk with a strong focus on quality improvement and benefits realisation.
The postholder will be expected to undertake regular horizon scanning of evidence based good practice and locally adapt where appropriate into delivery plans/practice.

The postholder will be expected to plan for and deliver on key milestones, professional and methodical manner. This would include contingencies and alternative actions required if milestones are not achieved, identifying interdependencies across projects/functions, risks and mitigation strategies, potential impacts on wider partnership, mapping resource requirements and escalations as appropriate.
Primary Care Networks
As part of work to develop the Local Care Networks, the post holder will work with local primary care leaders, Primary Care Networks, the GP Federation and the local contractor committees to support development and communicate the vision primary care transformation that is innovative, reduces variation, improves quality and ensures the sustainability of primary care for services and the local workforce.
Information Management
