Job summary

An exciting opportunity has arisen for a motivated and enthusiastic nurse to join our Cardiology Service as a Specialist Adult Congenital Heart Disease (ACHD) Nurse. Based across Eastbourne District General Hospital and Conquest Hospital, you will provide specialist care and support to adults living with congenital heart disease, working as part of a dedicated multidisciplinary team.

This established specialist role offers the chance to make a real difference to patients' lives while delivering high-quality, holistic and patient-centred care. We are seeking an experienced cardiac nurse with excellent clinical assessment, communication and organisational skills, alongside the vision and commitment to further develop ACHD services within the Trust.

The successful candidate will work across outpatient clinics and other clinical settings, supporting the delivery and ongoing development of specialist ACHD care. The service is supported by a local Consultant Cardiologist and visiting Consultants from specialist tertiary centres, ensuring access to expert multidisciplinary input and strong links with regional specialist services.

Main duties of the job

The successful candidate will manage a caseload of adults with congenital heart disease, providing specialist assessment, care coordination and ongoing support throughout their patient journey. The role includes assessing cardiovascular risk and patients with valve disorders; whilst working collaboratively with the physiology team within the valve clinic. Working closely with Consultant Cardiologists and a wide range of multidisciplinary professionals, including specialist teams at tertiary ACHD centres, the post holder will play a key role in ensuring the delivery of seamless, high-quality patient-centred care.

A key aspect of the role is empowering patients to understand and manage their condition through education and support. This includes providing advice on congenital heart disease, cardiac risk factor modification, healthy lifestyle choices and long-term health management. The ability to assess psychosocial needs and provide appropriate support and signposting is essential.
