Job summary
We are seeking a highly motivated, skilled, and autonomous Advanced Nurse Practitioner to join our clinical team. Working as an independent clinician, you will deliver high-quality, patient-centred care. You will manage same-day acute presentations, conduct triage, and look after patients with undifferentiated, undiagnosed, or chronic conditions. You will work closely with our GPs and wider clinical team to help streamline patient pathways.
You will be joining a fantastic and well established ANP team who are an integral part of this practice.
Main duties of the job
The successful candidate will act as an autonomous practitioner, working independently and collaboratively within the multidisciplinary general practice team. The post holder will provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems, making use of expert history taking, physical examination, and clinical decision-making to establish a diagnosis and management plan.
As an Independent Prescriber, you will manage a clinical caseload involving both acute presentations and the evaluation of chronic, long-term conditions. The role integrates advanced clinical practice with nurse leadership, mentorship, and evidence-based service development.
About us
Kirkham Health Centre is a well-established, patient centred GP surgery situated in the market town of Kirkham within the Fylde Coast area. We have a patient population of 9,800 patients, covering a mixed urban and rural area.
We own our own building and have an extensive primary care team comprising of 4 GP Partners,1 Salaried GP, 3 ANPs, 4 Practice Nurses, 1 HCA, 1 Nursing Associate, Clinical Pharmacist, Social Prescriber, Mental Health Practitioner and a supportive Administration and Reception team.
We pride ourselves on being a friendly and welcoming practice who are committed to delivering high-quality care to our patients. Our triage system coordinated by our Advanced Nurse Practitioner team allows for excellent access to services. We are active members of our local Primary Care Network, perform well in QOF and are rated "Good" by the CQC.

Job responsibilities
- Deliver a high standard of evidence-based care through face-to-face consultations, telephone triage, and home visits.
- Assess, diagnose, treat, or refer patients presenting with a wide range of acute, urgent, or chronic physical conditions.
- Function as an Independent Nurse Prescriber, safely prescribing medications in accordance with national frameworks and NICE guidelines.
- Request and interpret diagnostic investigations (e.g., blood tests, X-rays, scans) and formulate definitive treatment plans.
- Proactively manage a complex caseload of patients with long-term conditions (such as asthma, COPD, diabetes, and cardiovascular disease).
- Work collaboratively with General Practitioners (GPs), practice nurses, and administrative staff to ensure seamless patient care pathways.
- Act as a clinical supervisor and mentor to junior colleagues, practice nurses, and trainee nurses.
- Maintain meticulous, accurate electronic patient records in line with NMC guidelines.
- Participate in and lead clinical audits and quality improvement projects to optimise practice safety and effectiveness.
- Critically utilise evidence-based practice to update localised clinical protocols and workflows.
