Job summary
Care Navigator Butetown Medical Practice
We are a friendly, fun and supportive mixed-age team at Butetown Medical Practice, and we are looking for two enthusiastic people to join us as Care Navigators.
As a Care Navigator, you will be the first point of contact for patients, helping them access the right services and support for their individual needs. You will work closely with GPs, nurses, administrative staff and other members of the practice team to ensure patients receive the appropriate care and guidance.
Key responsibilities- Supporting patients to access the most appropriate health, wellbeing and community services.
- Listening to patients and understanding their individual needs.
- Providing information and signposting to relevant services and organisations.
- Working closely with the wider practice team and external services.
- Maintaining accurate and confidential patient records.
- Helping patients navigate the NHS and local community support available to them.
We are looking for people who are friendly, approachable, organised and compassionate, with excellent communication skills and a genuine desire to make a difference to our patients.

Main duties of the job

- Meeting and greeting patients in a friendly, welcoming and professional manner.
- Booking, cancelling and rearranging patient appointments.
- Dealing with patient enquiries in person, by telephone and via email.
- Answering telephone calls and directing patients to the appropriate service or member of the team.
- Managing incoming and outgoing emails and responding to queries appropriately.
- Scanning and uploading patient correspondence and documents onto the clinical system.
- Coding patient information and ensuring records are kept accurate and up to date.
- Maintaining patient confidentiality at all times.
- Working closely with the wider practice team to ensure patients receive a high-quality and efficient service.
- Providing general administrative and reception support as required.
