Job summary
Later Life Neighbourhood Service Tandridge are seeking an enthusiastic community mental health nurse to join the team.
As a Community Psychiatric Nurse, you will work within a neighbourhood team, partnering with statutory, voluntary, and community sector organisations to deliver a coordinated, person-centred approach to care.
The successful candidate will provide specialist mental health assessment, intervention, treatment, and ongoing support to patients within their own homes and community settings. You will act as a Care Coordinator/Lead Professional, where appropriate, ensuring high-quality care that promotes recovery, independence, and wellbeing.
You will be supported by an experienced multidisciplinary team and have opportunities to further develop your clinical knowledge and leadership skills.
Subject to reasonable adjustments in respect of any disability, candidates must possess a valid driving licence and be able to drive and have the unrestricted use of a car or motorcycle for work at all times including community based work.
Main duties of the job
Main Duties
- Undertake comprehensive mental health assessments and risk assessments.
- Deliver evidence-based nursing interventions and treatment plans.
- Act as a Care Coordinator/Lead Professional for individuals with complex mental health needs.
- Work collaboratively with patients, carers, families, and partner agencies.
- Promote recovery-focused and person-centred care.
- Contribute to safeguarding, risk management, and clinical governance requirements.
- Support service development and quality improvement initiatives.
- Demonstrate good communications skills and able to work collaboratively within a multi-agency team.
