Job summary

As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitate group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future. You report to the Safeguarding Lead, and will manage internal investigations and liaise with home authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can see your career grow and you achieve your career aspirations.
Main duties of the job
As a Social Worker, you play a pivotal role in changing lives for the better by aiding vulnerable people in securing brighter futures. Working part-time at Aderyn, you'll join a multidisciplinary team focused on providing high-quality step-down services for men with enduring mental health conditions. You'll manage complex caseloads, conduct social care and risk assessments, and facilitate group activities. Furthermore, you'll contribute to Integrated Care Record (ICR) meetings and aid service users in crafting viable future plans. Reporting to the Safeguarding Lead, you'll manage internal investigations and collaborate with home authorities during the admission and discharge processes, benefiting from training opportunities for professional growth.

As a Social Worker you will be:
- Referrals to LADO and Local Authority in line with safeguarding and assessments
- Educating Hospital staff about relevant safeguarding matters.
- Completing a full annual social care reassessment within the CPA format
- Facilitating access to welfare rights, housing, advocacy services and associated support services
- Participating in 1:1 supervision
To be successful in this role, you'll need:
- A Social Work degree
- Registered with Social Work England or applied for
- Experience within similar environment
- Experience with working with vulnerable adults

Location: Penperlleni, Nr Pontypool, Monmouthshire, Wales NP4 0AH
You will be working at Aderyn, a high quality step down service for men with enduring mental health conditions sets a sector-leading example of clear care pathways for service user with a focus on recovery. As well as a 17 bed rehabilitation unit there is also a 2 bed open rehabilitation cottage.
The unit is situated in a picturesque part of Wales with good transport links. You will working alongside a multidisciplinary team to provide service users with opportunities to improve their independence and prepare for an independent life in the community. Individual and collaborative therapeutic timetables are developed for each patient and individual psychological therapies are offered.
