Job summary

We are seeking an exceptional Nurse Practitioner (ANP) to join our dedicated multidisciplinary team and play a key role in delivering outstanding patient care.

This is an exciting opportunity to work autonomously across a diverse range of acute, urgent, chronic and complex presentations whilst helping to shape and develop services for our growing patient population.

Main duties of the job

The Role

As an Nurse Practitioner, you will be a highly skilled and autonomous clinician, providing expert assessment, diagnosis, treatment, prescribing and ongoing care for patients presenting with both acute and long-term conditions.

You will utilise clinical decision-making skills to manage differentiated and undifferentiated presentations, deliver same-day urgent care, undertake home visits where appropriate, and contribute to the continuous improvement of clinical services.

Clinical Practice

Assess, diagnose and manage acute, chronic and complex conditions

Provide same-day urgent care and clinical triage.

Independently prescribe medication.

Request and interpret diagnostic investigations

Develop personalised care plans and support patient self-management.

Leadership

Provide clinical leadership within the team.

Mentor and support junior clinicians and students.

Contribute to service development and quality improvement projects.

Education

Share specialist knowledge and expertise across the practice.

Support training and professional development of colleagues.

Research & Quality Improvement

Participate in clinical audits, governance activities and research initiatives.

Drive innovation and improvements in patient care.

About us

Millview Medical Centre is a busy, two site General Practice located in the rural village of Heckington as well as the growing Market town of Sleaford. The surgery has over 12,500 patients who have the opportunity to be seen at either site. We are a friendly, forward-thinking practice serving our local communities with pride.

We work closely with our Federation (K2) and host additional services such as a Health and Wellbeing Team, Clinical Pharmacists, MSK specialist as well as a number of outside agencies.
