Job summary

Jorvik Gillygate Practice is looking for an experienced and enthusiastic Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our established multidisciplinary team.

We are particularly interested in applicants who are qualified independent prescribers, although we would be happy to consider an experienced candidate who is currently completing, or committed to completing, their prescribing qualification.

This is a varied and autonomous clinical role, working alongside our GP, nursing, pharmacy and wider multidisciplinary teams to assess, diagnose, treat and manage patients presenting with a broad range of acute and ongoing health needs.

We would particularly welcome applications from clinicians with an interest or experience in Heart Failure, although this is not essential.

The successful candidate will work autonomously within their professional competence and scope of practice, with appropriate clinical support available.
