Job summary

As a Senior Clinical Nurse, youll provide safe and effective clinical care to patients transitioning between hospital and community settings. Youll work closely with NHS hospitals, healthcare professionals, patients and carers to ensure a joined-up approach to care.

Main duties of the job

Alongside maintaining your own clinical practice, you'll lead and support a small team of Clinical Nurses, helping them deliver excellent patient care and achieve agreed service standards.

Job responsibilities

Your responsibilities will include:

  • Delivering clinical care and patient education in patients homes and virtually
  • Supporting patients, families and carers throughout their treatment
  • Working with NHS hospitals and multidisciplinary teams to support safe discharge and ongoing care
  • Leading, coaching and supporting a small clinical team
  • Monitoring clinical quality, patient outcomes and service KPIs
  • Maintaining accurate and timely clinical documentation
  • Managing and escalating clinical incidents and adverse events appropriately
  • Supporting training, development and performance within the team
  • Participating in the on-call service as required

  • Minimum 2 years post-registration experience

  • Experience with CVAD, parenteral nutrition, or similar therapies
